Children's pre-exam questionnaire
Please complete form below at least 48 hours prior to appointment.
Due to new guidelines set out by the States of Guernsey we would ask you to under go a Lateral Flow Test on the morning of your appointment and to wear a mask when entering the practice.
If your child is experiencing any Covid symptoms, please do not attend the practice.
This questionnaire is intended to help us prepare the appropriate tests for your child and reduce the time that they are kept waiting in the practice.
Please only fill in those areas which you are comfortable to do so. We can assure you that it will be dealt with in a confidential manner.
